What's Included
1:1 Sessions
Visits will occur every two to four weeks to review blood sugar logs, challenges, and wins with a Diabetes Expert. Initial visits will be every two weeks to build confidence and will be decreased in frequency as needed.
Consistent Communication
Receive personalized support via messaging when you upload your blood sugars outside of our 1:1 visits. Ask questions on the Get Healthie App and receive support Monday-Friday from your personal CDCES.
Ongoing Support
Weekly food log and blood sugar review where we can troubleshoot any concerns with your blood sugar together.
